Music glitch on HTC Evo phone, how can I fix this?

Sometimes when I play music through my headphones the song suddenly starts skipping forwards or backwards, faster and faster. Then my phone randomly opens up Voice Dialer and calls people, though I didn't even click on anything. It's really annoying, and I don't know why it does this. How can I fix this?
