What's the difference between refresh rate and frame rate?

The frame rate of video on broadcast TV is 30 frames per second, but actually 60 interlaced fields per second, so what is the refresh rate? If a TV paints the pixels on the screen line by every other line 60 times a second, then what does it mean to have a 90 hertz refresh rate?
